Include Primary DX Description Column

For all reports that include a column for Primary DX, also include the Primary DX description to allow for ease of reporting of high volume DX codes.

Include date of injury with counter under doctor icon in GO

Include date of injury with counter (similar to date of surgery with counter) under doctor icon in GO. If we are treating a conservatively managed patient, it is helpful to know an exact time frame since injury when considering progression.

Add click box to mark whether patient is on a functional maintenance program on all notes.

This will let each supervising therapist know if a patient is on a maintanence program without having to reach through evaluations or recerts.
