We would like to add our logo, and some boilerplate text to the schedule we hand to patients. Is that editable in that way, or should we create a new report to do patient schedules with our logo and boilerplate?

CPT codes for cash pay (guarantor)

Cash rates Wants all of the CPT codes listed, but have a cash rate- i.e. it writes it off. Stops billing after the cash rate, or adjusts off. Lists CPT code Submit for reimburshment from insurance companies and companies like Medishare and AFLAC.
