Referring MD field additional details besides name needed

When entering referring MD from drop down list, there is no way to distinguish the difference between 2 contacts with the same name. Need additional fields to be visible (i.e. NPI, Fax #, or Notes) so we can determine which name to choose from list.

Unable to activate a Patient in IGO if a case is created when inactive
