Add Functional Outcome Measures to reports

Could I please request that this information is added to the Case Information Report? Functional Outcome Measure Name, Functional Outcome Score, Date of Visit/Capture. Or the Appt research report, I'm not sure what would be best for it to appear.

Essentially it shows the patient, the case ID of the patient, the date of the visit, the outcome that was captured, the type of visit that the patient was seen that day (IE, Progress Note) etc., the score of the outcome measure. Oh, also Program Type
