Report on ALL adjustment codes

There is not currently a report that shows the reason codes and amounts for contractual adjustments.

We would like to put in a feature request. The reason we would use this is most payers put their reasons in this bucket, so we want to look for issues/trends.

Also, in the therapy world there are a lot of payers that pay a flat rate or a 4 limit on units. The rest then are returned as contractual adjustments. We want to be able to report off those codes but currently aren't able to report off these, so instead have to manually research patients w/ these insurances.
