A way to track and report on the Frequency and Duration per the Referring MD POC

I can't find a report that would give me the Plan of Care Freqency for our PTs. For example, if a PT had 20 evaluations this month, I would like to know if the freqency they chose in the plan of care is twice a week or three times a week. Can you help me find a report with this info in it or tell me how to pull it out?
