Multiple Referral Sources are needed for WC payers

Request to add another Referral Source field; the ability to see referral details for case managers and insurance adjusters is vital to our workflow. Work comp patients have case manager AND adjuster, contact information needs to be visible for phone calls. These contacts will also need to be fax recipients, and ideally reportable.
