Medicare signatures by primary Dr

We are struggling with our Medicare and Medicaid POC's that we need to send to primary Dr vs referring Dr and then the recert POC's as well as only the referring shows up on the eval and in the signature box, is there a way to show primary and referring and or let us choose as the Medicare and Medicaids need to be signed by primary Dr. Thanks,
