Health Care Proxy / Power of Attorney

We would love to have a section of the demographics where we could enter the name/phone number/address of the patient's Health Care Proxy or Power of Attorney and maybe a "yes/no" toggle for whether we have the paperwork for this on file in the attachments.

Please let me know if any more information would be helpful.
