Automatically assign new waistlist entry to clinic in which we're scheduling Merged

When we add someone to the waitlist, instead of having to assign a clinic, it should automatically assign it to the clinic in which we’re scheduling OR to the therapist’s home clinic (which is really the same in most cases)
