Remove unused issues from the Patient with Issues Listing

Please remove the following items for the patients with issue listing. The Clinic does not use these, making the listing very long and hard to get to the items that are missing.

Remove these for all payers primary, secondary, and tertiary

No relationship specified for primary payer.
No subscriber specified for primary payer.
Primary Payer Insurance Member ID, Policy ID and Claim Number are all blank.
