Since insurance auditors are asking for treatment rationale for billing Ther. Ex, Neuro reed, and Ther. Activities, I would like to suggest a drop down menu that would pop up when each of these codes are used so that you could customize for each patient the rationale of why coding with that specific code and it could be customized by PT for that patient. For example: If I am doing single leg balance and have chosen the Neuro reed code, when 97112 is used, it would have a drop down menu that the PT could choose why they are billing Neuro reed. The drop down might have a list of the following: proprioception, balance, core stabilization, scapular stabilization, etc. When any of these are chosen it would populate in the narrative that the exercise chosen was done for balance in this case. This would force PT's to include this in documentation so they don't forget to document rationale for each exercise and therefore, make documentation much more efficient.
