request for the KX modifiers to show in the chart notes to match the claim documentation

Suggest that we request a feature in InsightGO that automatically adds text to the chart note rather than showing the modifier on the chart note.

If the KX modifier is applied at sign off: "Our calculations indicate that cumulative allowed charges for this patient have exceeded the annual therapy threshold. At sign off the treating therapist stipulated that continued treatment is medically necessary."

If the patient reaches the threshold and the KX modifier is not applied at sign off: "Our calculations indicate that cumulative allowed charges for this patient have exceeded the annual therapy threshold, but continued treatment is not medically necessary. The patient has been notified that Medicare will not cover continued treatment that is not medically necessary."
