Ability to edit the summary section chart note at sign off

Is there a way that we can add in a field when e-faxing notes to clinicians that we can edit the last page that the physicians/clinicians have to sign. The page is sent blank and we have no to modify the fields without printing off the note and faxing it in or leave it blank which is confusing to the MD
