Add reason field for deleting appointments

We would like to track the reason appointments are being deleted. Is it possible to add a reason field that need to be filled in prior to deleting an appointment.

adding logo and boilerplate to the patient schedule

We would like to add our logo, and some boilerplate text to the schedule we hand to patients. Is that editable in that way, or should we create a new report to do patient schedules with our logo and boilerplate?

Customize and add messages to Patient Schedule printout

Is it possible to add a message on the page that would print off for patient appointments? We would like to add a 1 line disclaimer. " Therapist is subject to change"
